NHS Summary Care Records

The NHS is introducing an improved way of storing and managing important medical information electronically, called Summary Care Records. We are taking part in this scheme which means that, in the future, and with your consent, key information from your GP record will start to become available electronically to NHS staff treating you out of hours or in an emergency.

Summary Care Records will contain key information about your health: details of your allergies, your current prescriptions and whether you have had any bad reactions to medicines.

All registered patients over the age of 16 receive a letter, information leaflet and an opt- out form through the post which gives you more information.

If you would like to have a Summary Care Record you don’t need to do anything.

If you do not want to have a Summary Care Record, you can opt out by filling in the opt- out form included in your information pack.  You can either post it back to the freepost address on the back of the form or bring it into the surgery.

If you have any questions about Summary Care Records please contact the NHS Heywood, Middleton and Rochdale Patient Advice and Liaison Service (PALS) on Fr eephone 0800 328 3166 (Monday - Friday, 9.00am - 3.00pm) or email  mailto:pals@hmr.nhs.uk Alternatively you can contact the national SCR information line on 0300 123 3020.

Zero Tolerance Policy

Staff treating patients have the right to work free from any threat or fear of abuse or violence.

The NHS nationally have agreed a Zero Tolerance Policy. If any patient is abusive or violent towards any member of staff or any other persons on the practice premises, the practice retains the right to have the patient removed from our list

New rooms

We have  finished our new extension. We now have 4 extra new rooms to see patients , a second waiting area and a second conference room.

You can wait in the new waiting area if you are seeing a doctor or practice nurse in the new extension.

Automated book-in system This is a quick and easy way to book yourself using a touch screen in reception when attending the surgery for an appointment. This will free the receptionist to deal with other queries and to answer the telephone. A member of staff will be happy to assist if you have any difficulties.

Blood pressure, weight and height machine

We have a machine located in the corridor for patients to use.  It can be used to check BP, weight, height and BMI. Please ask for a token from the receptionist and follow the instructions. It will print 2 copies, please hand 1 copy to a receptionist or the GP/nurse in case any action is required.
